Let us know the other category of medication helpful in case 1st category fails to give you immediate recovery. It's relieving that we have many alternative options. The other options include if antacid isn't enough-
1. H2 Blocker
2. Proton pump Inhibitor
3. Prostaglandins
Let us start with H2 Blocker.
▶▶ What does H2 Blocker mean??
H2 Blocker are also known as H2 receptor Blocker or H2 antagonist. In our body, there are variety of receptors which are protein molecules that receive chemical signals from outsides and when a drug molecule or any chemical substance bind to a receptor, it causes cellular or tissue response. In H2 Blocker, the letter "H" represents Histamine. Histamine is an organic nitrogenous compound is released by the cells in response to injury and as part of the local immune response to invading bodies and triggers inflammation as well as performing several important functions in the bowel.
It is released from mast cells, enterochromaffin-like cells, and neurons. Its various actions are mediated by histamine receptors H1, H2, H3 and H4. The histamine receptor H2 belongs to the rhodopsin-like family of G protein-coupled receptors. It is an integral membrane protein and stimulates gastric acid secretion. It also regulates gastrointestinal motility and intestinal secretion and is thought to be involved in regulating cell growth and differentiation.
Histamine stimulates the secretion of gastric acid by action on H2 receptors, which are found in the parietal cells of the stomach. H2 blockers are a group of medicines that reduce the amount of acid produced by the cells in the lining of the stomach.
▶▶ Common H2 Blockers
There are different types of H2 blocker with different names and brands. Most work equally as well. Side effects may vary from drug to drug.
- Famotidine
- Cimetidine
- Ranitidine
- Nizatidine
The selective H2 blockers were first developed in the early 1990s by Sir James Black, who subsequently received the Nobel Prize for his work developing selective receptor antagonists for clinical use (including the beta blockers as well as the H2 blockers). The initial H2 blocker approved for use in the United States was cimetidine (1977), which was followed by ranitidine (1983), famotidine (1986), and nizatidine (1988). All four of these agents are available by prescription and as over-the-counter oral formulations. Intravenous and intramuscular forms are available for cimetidine, ranitidine and famotidine.
▶▶ How do H2 Blockers work??
When the naturally produced mucous barrier that protects the lining of the stomach from being worn away in response to acid is damaged allowing the acid to damage the stomach, causing the ulcer. . In some people, there may be a problem with the muscular band at the top of the stomach (the sphincter) that keeps the stomach tightly closed. This may allow the acid to escape and irritate the gullet (oesophagus). This is called acid reflux, which can cause heartburn and/or inflammation of the gullet (oesophagitis).
The letter H in their name stands for histamine. Histamine is a chemical naturally produced by certain cells in the body, including cells in the lining of the stomach, called the enterochromaffin-like cells (ECL cells). Histamine released from ECL cells then stimulates the acid-making cells (parietal cells) in the lining of the stomach to release acid.
When you take an H2 receptor blocker, the active ingredients travel to specific receptors on the surface of the stomach cells which is H2 receptor that release acids. What H2 blockers do is competitively inhibits histamine at the H2 receptors and stop the acid-making cells in the stomach lining from responding to histamine leading to a reduction in secretion of gastric acid in your stomach.
By decreasing the amount of acid, H2 blockers can help to reduce acid reflux-related symptoms such as heartburn. This can also help to heal ulcers found in the stomach or in part of the gut (the duodenum).
▶▶ What's the difference between Antacid and H2 Blockers??
Antacids are substances that tend to counteract the acids of the stomach. Hence, they are more or less bases (the opposite of acids) or basic substances. When the stomach's overall acid pH goes dangerously down, the person can feel a little pain at the epigastric area because of too much acidity. This is where antacids come in. They elevate the gastric pH to restore the ideal pH level that is just acidic enough.
On the other hand, an H2 blocker’s mechanism of action is very much different from that of antacids. It actually blocks the action of histamine on the gastric wall. This histamine supposedly acts on the wall’s parietal cells making it produce more acid. By blocking it, H2 blockers tend to reduce acidic secretions from the said cells. That is why these drugs are technically known as H2-receptor antagonists.
✅In brief, Antacids raise the stomach’s pH level through an acid neutralizing effect to counter acidity whereas H2 blockers inhibit the action of histamine to prevent some stomach cells from producing too much acid.
✅Antacids have a serious issue when given alongside tetracyclines whereas patients using H2 blockers must carefully monitor the blood levels of some drugs administered with it like lidocaine and warfarin since H2 blockers tend to increase the serum levels of other drugs.

▶▶ How long the Effect of H2 Blocker persists??
Studies show that, H2 receptor blockers decrease stomach acid secretions over a 24-hour period by 70 percent. By reducing the amount of acid in the stomach, any damaged tissues are allowed time to heal.
The effect of H2 blockers is largely on basal and nocturnal acid secretion.
▶▶ When to take H2 Blockers??
H2 blockers are most often taken by mouth. You can get them in the form of tablets, liquids, or capsules.
- These medicines are most often taken with the first meal of the day. In some cases, you may also take them before your evening meal.
- It takes 30 to 90 minutes for the medicines to work. The benefits will last several hours. People often take the drugs at bedtime, as well.
- Symptoms may improve for up to 24 hours after taking the drug.
▶▶ Common Side Effects and way of administration of H2 Blockers

- Headache

- Breast swelling or tenderness (in men)
- Joint or muscle pain

- ◼Deficiency of Granulocytes a Type of White Blood Cell

- ◼Breast Tenderness
- ◼Enlarged Breasts

- ◼Sexual Problems
✱ ✱ Do NOT use cimetidine if:
- You are allergic to any ingredient in cimetidine
- You are taking medicine for cancer (nitrosoureas)
- You are taking cisapride, dofetilide, or a nitrosoureas (eg, carmustine).
❄ How to use cimetidine:
Use cimetidine as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Take cimetidine by mouth with or without food.
- Do not take an antacid within 1 hour before or after you take cimetidine.
- If you also take cephalosporin (eg, cephalexin), itraconazole, or ketoconazole, take it at least 2 hours before taking cimetidine. Check with your doctor if you have questions.
- If you miss a dose of cimetidine and you are taking it regularly, use it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not use 2 doses at once.
🔴 Common side effects of Famotidine
The adult side effects for this drug are slightly different from the side effects for children.
- Adult side effects can include:
- headache
- dizziness
- constipation
- diarrhea
- Children under one year of age may also experience:
- agitation
- unusual restlessness or crying for no clear reason
If these effects are mild, they may go away within a few days or a couple of weeks. If they’re more severe or don’t go away, talk to your doctor or pharmacist.
✱ Serious Side Effects of Famotidine
Serious side effects and their symptoms can include the following:
- Heart rate and rhythm problems. Symptoms can include:
- dizziness
- fainting
- shortness of breath
- irregular heart rate and rhythm
- Severe muscle problems. Symptoms can include:
- unusual muscle pain that you cannot explain
- weakness
- fever
- Neurological problems. Symptoms can include:
- agitation
- anxiety
- depression
- trouble sleeping
- seizures
- sexual problems, such as decreased sex drive
- Liver problems. Symptoms can include:
- unexplained or unusual weakness
- decrease in appetite
- pain in your abdomen (stomach area)
- change in the color of your urine
- yellowing of your skin or the whites of your eyes
- Skin problems. Symptoms can include:
- blisters
- rash
- mouth sores or ulcers
❄ How to use Famotidine:
Prescription famotidine comes as a tablet and a suspension (liquid) to take by mouth. It is usually taken once daily at bedtime or two to four times a day. Over-the-counter famotidine comes as a tablet, a chewable tablet, and a capsule to take by mouth. It is usually taken once or twice a day.
To prevent symptoms, it is taken 15 to 60 minutes before eating foods or drinking drinks that may cause heartburn. Follow the directions on your prescription or the package label carefully. Take famotidine exactly as directed. Do not take more or less of it or take it more often or for a longer time than prescribed by your doctor.
Shake the liquid well for 5 to 10 seconds before each use to mix the medicine evenly.
Swallow the tablets and capsules with a full glass of water.
Thoroughly chew the chewable tablets before swallowing them. Swallow the chewed tablet with a full glass of water.
Do not take more than two tablets, capsules, or chewable tablets of over-the-counter famotidine in 24 hours and do not take over-the-counter famotidine for longer than 2 weeks unless your doctor tells you that you should. If symptoms of heartburn, acid indigestion, or sour stomach last longer than 2 weeks, stop taking over-the-counter famotidine and call your doctor.
✴✴ You can take Famotidine along with Antacid.
🔴 Common side effects of Ranitidine
- headache (may be severe)
- drowsiness, dizziness
- sleep problems (insomnia)
- decreased sex drive, impotence, or difficulty having an orgasm
- swollen or tender breasts (in men)
- nausea, vomiting, stomach pain
- diarrhea or constipation.
✱ Serious Side effects of Ranitidine
- chest pain, fever, feeling short of breath, coughing up green or yellow mucus
- easy bruising or bleeding, unusual weakness
- fast or slow heart rate
- problems with your vision
- fever, sore throat, and headache with a severe blistering, peeling, and red skin rash
- nausea, stomach pain, low fever, loss of appetite, dark urine, clay-colored stools, jaundice (yellowing of the skin or eyes).
❄ How to use Ranitidine:
Take this medication by mouth with or without food, usually once or twice daily or as directed by your doctor.
It may be prescribed 4 times a day for some conditions. If you are taking this medication once daily, it is usually taken after the evening meal or before bedtime.The dosage and length of treatment are based on your medical condition and response to therapy. In children, dosage may also be based on body weight. Follow your doctor's instructions carefully.
You may take other medications (e.g., antacids) for your condition as recommended by your doctor.Take this medication regularly in order to get the most benefit from it. To help you remember, take it at the same time(s) each day. Do not increase your dose or take it more often than prescribed.
If you are using nonprescription ranitidine for self-treatment of acid indigestion or heartburn, take 1 tablet by mouth with a glass of water as needed. To prevent heartburn, take 1 tablet by mouth with a glass of water 30-60 minutes before eating food or drinking beverages that cause heartburn. Do not take more than 2 tablets in 24 hours unless directed by your doctor. Do not take for more than 14 days in a row without talking with your doctor.Inform your doctor if your condition does not improve or if it worsens.
🔴 Common side effects of Nizatidine
- Headache, dizziness
- Mild rash
- Diarrhea
- Runny or stuffy nose, sore throat, cough.
✱ Serious Side effects of Nizatidine
- Pale skin, feeling light-headed or short of breath, rapid heart rate, trouble concentrating.
- Unusual bleeding, purple or red pinpoint spots under your skin;
- Skin rash, bruising, severe tingling, numbness, pain, muscle weakness.
- Fever, confusion.
- Jaundice (yellowing of the skin or eyes).
❄ How to use Nizatidine:
Use Nizatidine as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Nizatidine maybe taken with or without food.
- Continue to use nizatidine even if you feel well. Do not miss any doses.
- Antacids may be taken as needed to help relieve stomach pain, unless your health care provider instructed otherwise.
- If you miss a dose of nizatidine, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once.
▶▶ Major Drug Interaction of H2 Blockers
- Cimetidine inhibits multiple forms of cytochrome P450 (CYP1A2, CYP2C9, CYP2D6 & CYP3A4). Cytochrome P450 is an important enzyme which function to metabolize potentially toxic compounds, including drugs and products of endogenous metabolism such as bilirubin, principally in the liver.
- Hence the half-lives of drugs metabolized by these pathways may be prolonged. Examples: warfarin, theophylline, phenytoin, lidocaine, quinidine, propranolol, metoprolol, tricyclic antidepressants, benzodiazepines, calcium channel blockers, sulfonylureas, and ethanol.
- Ranitidine binds 4-10 times less avidly to cytP450 than cimetidine.
- H2 blockers also compete with drugs for renal tubular secretion via P-glycoprotein (e.g. procainamide).
▶▶ Who cannot take H2 Blockers??
H2 blockers may not be suitable for people with kidney problems or for pregnant or breast-feeding mothers. A full list of people who should not take H2 blockers is included with the information leaflet that comes in the medicine packet. If you are prescribed or buy an H2 blocker, read this to be sure you are safe to take it.
Note: taking some H2 blockers can affect how well other medicines work. In particular, tell your doctor if you are taking the blood-thinning medicine warfarin or a medicine for epilepsy, called phenytoin (Epanutin®). You should also tell your doctor if you take theophylline, a medicine commonly used to treat asthma or chronic obstructive pulmonary disease (COPD).
